Medication Reconciliation Avoids Dangerous Errors

Medication reconciliation is a comprehensive review of a patient’s active medications during care transitions. It is a strategy that enables healthcare providers to make better prescribing decisions for patients. The Joint Commission (TJC) added medication reconciliation to the National Patient Safety Goals (NPSGs) over a decade ago with the intent to improve medication safety across the continuum of care. The addition of medication reconciliation as an NPSG created an awareness of the benefits of the reconciliation and formalized the process to bridge the gap between a patient’s medication history and acute treatments. This course provides information about the medication reconciliation process and requirements, as well as the importance of medication reconciliation in improving patient safety. The goal of this course is to inform providers, nurses, and pharmacists with knowledge about medication reconciliation.
